Dear Panama Canal Area Benefit Plan (PCABP) Provider

ISSUE DATE AND VALIDITY PERIOD

This allows for verification of the prescription’s validity and streamlines the authorization and dispensing of medications.

Must include:

• Regular prescriptions are valid for 30 days.

• Antibiotic prescriptions: 3 days.

• Prescriptions for controlled substances: 5 days.

DIAGNOSIS

Supports the medical necessity of the treatment and facilitates clinical and administrative evaluation.

PLEASE NOTE

Prescriptions without a date or diagnosis may cause delays in the validation process.

In addition, all prescriptions must include:

• The patient’s full name.

• Medication, dosage, and frequency.

• Indicate when the prescription is for long-term use.

• The physician’s signature and stamp.
